Insurance Accepted & Schedule of Fees
Insurance:
EVE Counseling & Coaching and Heidi W. Neuberg, MS, LPC, LRC is an in-network contracted provider for Qualcare, Multiplan, and the Ceridian Employee Assistance Program. In addition, many clients find that our services are covered through their health insurance plan’s out-of-network benefits. Your counselor can provide a monthly statement that you can submit to your insurance provider if you have out-of-network benefits. We recommend that you contact your insurance company prior to your initial appointment with your counselor so that you are aware of your coverage and financial obligation such as co-payments.
If you decide to use out-of-network benefits, please be aware that health insurance companies often require a statement of diagnosis of a mental health condition from your Licensed Professional Counselor before they will agree to reimburse you for counseling services. Any diagnosis made will become part of your permanent insurance records.
Fees:
Payment is made to EVE Counseling & Coaching at the time of service by check or cash.
Schedule of Fees for Self-Pay or Out of Network Clients:
- Individual Counseling, initial appointment, 75 minutes: $80
- Individual Counseling, follow-up appointment, 50 minutes: $65
- Eye Opener or Grab-and-Go 30-minute group: $50/month for two group sessions
- WIN: Women In Transition 90-minute group: $25
- WE: Women Embracing Empowerment 90-minute group: $25
Discounts: Purchase a three month Eye-Opener or Grab-and-Go Group package for $150 and you will receive a $25 credit toward a future counseling service.
EVE Counseling & Coaching reserves two appointments for pro-bono counseling for women. Currently our pro-bono appointments are occupied. However, if you would like to be placed on our waiting list for pro-bono counseling services please call 973-998-0333.
